What is Hospice?

Hospice is … care that improves the quality of life for those faced with advanced illness. It is providing comfort, dignity and support compassionately through specialized and uniquely trained personnel. They are dedicated and devoted to honoring and supporting the patient’s individual journey with love, care, and tenderness.

Hospice is the preferred choice of healthcare delivery for the terminally ill or those in their last 6 months of life. Horizon Hospice provides: Skilled nursing visits, pain and symptom management, equipment, medications related to illness, personal care assistance, incontinent supplies, education to end of life, 24 hour on call support, counseling services, assistance with community resources, spiritual support, companionship to patient and family, as well as bereavement support and counseling after patient’s death. Hospice allows the patient and their family to remain together in the comfort and dignity of their home…where ever that may be and focus on living. Hospice treats the person, not the disease; it focuses on the family, not only the individual; and emphasizes the quality of life, not the duration.

Symptoms that indicate hospice eligibility may include: Weight loss, poor appetite, shortness of breath, and increased need for assistance with personal cares, falls, frequent ER visits, change in responsiveness, weakness/fatigue, pain, edema, chest pain, frequent infections, and low oxygen levels.

When is it appropriate to ask about hospice? How does it begin?

You don’t have to wait until a physician brings up hospice to ask about it. The best time to learn about hospice and ask questions is before such services might be needed. Hospice care is available to people who are expected to live six months or less, no longer wish to pursue a cure and want to receive hospice care. Hospice care begins with a referral – usually from someone’s primary physician – but referrals can also be suggested by family members, friends, faith leaders, or health professionals. Hospice usually begins within 48 hours after a referral, but services may start faster in an urgent situation. For example, if someone is in a hospital, hospice care may be arranged to start upon discharge from the hospital.
