Pediatric Private Duty Nursing

Horizon Home Health is a contracted provider for pediatric private duty nursing for all of the Utah Medicaid Agencies, including Traditional Medicaid, Select Health Community Care, Healthy U Medicaid, and Health Choice. We are also contracted with most commercial insurances who offer private duty nursing as a benefit. Whether the child qualifies for 8 hours a day or 24 hours a day of nursing care, our skilled nurses go day and/or night to make sure the child’s needs are met, allowing the family to rest assured that their child is in good hands.

Pediatric Respite Care Services

Horizon Home Health is a provider for Utah Medicaid’s Technology Dependent Waiver (Travis C Waiver) and the Medically Complex Children’s Waiver (MCCW). Both of these waiver programs allow families to have a respite from their child’s demanding medical needs by having a skilled nurse care for the child in the home. Whatever the amount of respite hours that the child qualifies for, and whichever shifts the families choose, our nurses can help!

Pediatric School Nursing

Horizon Home Health is a contracted provider for 1:1 school nursing services for students in the Canyons, Jordan, Alpine, Provo, and Nebo school districts. Our nurses will accompany the child to school, caring for their medical needs so that the entire team at the school can focus on their educational needs.
