Scott Woolley

About our CEO

Scott B. Woolley grew up in California, right before High school his family moved to Utah and has lived in Utah since. While going to school at BYU studying finance,  he was the manager of his parents Pharmacy. He later received his MBA  at BYU with a emphasis in healthcare administration with high honors ( top 5% in his class).

Scott has worked in the Healthcare industry for more than 22 years. He started his home health career as an administrator for McKay-Dee Home Health.During this time he oversaw home health operations for Utah, Wyoming and Idaho for Intermountain Healthcare.

In 1995, Scott started Heritage Home Care Services in conjunction with Avalon Health Care. He was instrumental in growing operations to include the entire State of Utah, parts of Northern Arizona, and Western Colorado. In 2005, the Utah business was sold to Gentiva, Scott kept ownership of the Arizona Business. At the time of the sale Heritage was the largest home health provider in Utah.

In 1996, Scott reacquired the hospice program from Odyssey Healthcare and less than five years later they purchased its Utah Home Health program back from Gentiva. Since that time Horizon has expanded to include pediatric services and it is the largest provider of pediatric services in Northern Utah.

Scott has been involved in the healthcare community since he graduated from BYU in 1987. He is committed to the health and well-being of the community.

Spencer Woolley

About our CFO

Spencer Woolley is the Corporate Finance Officer and in addition to his ownership duties, he is specifically responsible for Business Development, Operations and Finance.   His background includes a Master’s Degree in Business Administration and Senior Management experience in Customer Service, Accounting, Finance, and Operations at large corporations such as WordPerfect, Novell, and Ancestry.com.   His broad range of experience and skills provide analytical and efficiency excellence to the Horizon business model.   As a Utah native he desires to strengthen its economy and business environment by building a strong company that generates local employment opportunities and keeps corporate profits in our communities.

About our Director of Nursing in Pediatrics:
TBU, RN, BSN

A graduate of the College of Eastern Utah, Utah Valley University, and Brigham Young University, Alli began her nursing career in 1997 in the Newborn ICU at Utah Valley Hospital, where she gained a lifelong love of caring for babies and children. From there she worked as a pediatric case manager until 2008 when she started doing respite care for her niece, who had a trach and ventilator. It was during these years taking care of her niece that she really came to understand what pediatric home care was all about. She observed the other agency and nurses in the home, listened to all the feedback from her brother and sister-in-law, and loved taking care of her sweet little patient. When her angel niece passed away in 2012, she began work at Horizon Home Health as a school nurse, respite nurse, and private duty nurse. In 2014, she became the director of the pediatric department. Alli knows what our patients and families need and is passionate about providing them with the best possible care!

About our Assistant Director of Nursing in Pediatrics:
TBU, APRN, PNP-C

Talena began her nursing career on an adult med/surg unit in 1989. She comes with experience in telemetry, med/surg, and cardiovascular nursing. The last six years of her nursing career have been in pediatrics. She developed her love of the pediatric population when she worked with a pediatrician in an outpatient clinic in CA. There she began schooling to receive her Pediatric Nurse Practitioner certification. Talena earned her degrees from George Mason University, BSN, University of Phoenix, MSN, and Maryville University, PNP. She is happy to be working with such awesome patients and their families. She know considers Utah her home after living here for the last three years. She has been married for thirty years and they have four children, two grandsons, and a beautiful Golden Retriever.
